What is the purpose of the survey?
The hospital will use the results of this survey to evaluate the standard of care provided to its patients and whether or not this standard meets patients’ expectations.
Do I have to take part?
Your participation in the survey is completely voluntary and you can withdraw from the survey at any time without your rights being affected in any way.
What would I have to do?
To participate, please complete a questionnaire. A FREEPOST envelope is available to facilitate the return of the completed survey questionnaire. No stamp is needed. The questionnaire should take around 15 minutes to complete.
If you do not wish to participate, please tick the box on back page of the questionnaire and return the incomplete questionnaire in the FREEPOST envelope provided – this will help to ensure that we will not contact you again in the future with any reminder correspondence.
If you choose not to take part, it will not affect the care you receive from this hospital in any way in the future. If we do not hear from you in 1‐2 weeks, we will send you a reminder.
How will my response be kept confidential?
You have been given a unique number just for this survey so that your name and address does not appear on the questionnaire. At no point will your name and address be linked to your responses for this survey.
Your contact details will only be used to send you this survey. The ISQSH will process your answers in confidence and keep them separate from your contact details.
The ISQSH will ensure your answers are kept confidential if you decide to participate and will maintain all information supplied in accordance with the most recent Data Protection Legislation.
I had a good experience in hospital, do I need to take part?
Patients regularly receive good quality care and like to acknowledge the staff that cared for them. If this has been your experience, you can also use this opportunity to let those people know that they are doing a good job. The findings of the survey will be used to help the hospital you attended learn from their patients’ experience and to use the feedback to improve their services. We want to hear all patients’ views, good and bad. This is your chance to have a say, so please avail of your opportunity to do so.
